We will be glad to refill prescriptions for active patients that are currently under our practice's care

Please contact the appropriate medical assistant for the doctor prescribing you your medication 

Email or phone are fine. 

Please provide the prescription request, your Date of Birth, your contact number and your pharmacy number 

Our medical assistants can be reached by email by clicking here. 

Please submit your requests during the weekdays from 8:30-5pm each day.  We do not refill prescriptions outside these hours. 

Finally, there are some medications which cannot be called in and require hand-written prescriptions, including Percocet and Oxycontin.
