First Appointment: What to Bring


We want your visit with us to run smoothly and efficiently. You can help us by bringing the following:

  1. Your insurance card and your photo ID
    Though we are not in network with any provider other than Medicare, we will submit your claim for you. See our billing policies for further information.
  2. Condition profile
    If you have been seen elsewhere, please bring a brief description of your condition with a list of tests and treatments you have received
  3. Imaging studies
    Relevant X-rays, other studies, reports, and test results obtained elsewhere.
  4. List of any medications
    On the list, please include name of medication, dosage, and how often you take the medication. Please also include supplements and over the counter medicines that you take on a regular basis. See our convenient first appointment checklist WOSM for an example of a medication list.
  5. Comfortable clothing
    Please wear clothes that allow you to show the injury site easily. For women with an upper extremity injury, a sports bra or tank top without under-wires will not interfere with X-rays. Those with a lower extremity injury should wear or bring shorts.

Again, please bring:

  • Brief description of your problem
  • List of any tests and treatments you have received
  • Insurance card
  • Photo ID
  • Relevant Films, X-rays, MRIs, CTs, and any other studies or reports
  • Medications List
  • Comfortable clothes, Sports Bra, Shorts
